Use this form to schedule and register your upcoming class with us.

This form is an integral part of your class with Dayo Scuba. We use this form EXCLUSIVELY to set up and schedule our classes. No one is confirmed in a class until this form is filled out and then confirm by a staff member. You can fill his form out at home, office, smart phone or even while in our shop. Rescheduling is often necessary but certainly can cause problems, so please read and understand our scheduling (and rescheduling) policy.
